Sciatica Causes
It is essential to understand that sciatica itself is not the name of a disease but rather it is condition caused by a set of other conditions. Sciatica is generally caused by these primary conditions:

Firstly we have Herniated Discs which is sciatica caused by the pressure from a herniated or protruding disc on the sciatic nerve. A herniated disc is simply a disc that has been displaced due to external pressure from an accident or by constant pressure due to muscle imbalance. This can be the cause for sciatic pain but it is equally important to note that many people with herniated discs may not experience any pain at all.

Then sciatica can be caused by Piriformis Syndrome. The piriformis muscle exerts pressure on the sciatic nerve when muscle imbalances cause the joints in the pelvic areas to be pulled out of place and thus changing the seat of the piriformis muscle, which in turn puts pressure on the sciatic nerve.

Spinal Stenosis is the narrowing or constriction of the space in the spinal canal causing muscle imbalances that tug at your hip and spine and cause them to become positioned awkwardly causing an irritation due to constant contact and pressuring the sciatic nerve.

Isthmic Spondylolisthesis
This is when a vertebra is dislocated from its normal position and can start to pinch the sciatic nerve.

If you have noticed all these conditions have a common denominator: muscle strain or muscle imbalance. A muscle injury can occur anywhere and it means that your muscle flexibility is reduced and there is pain in most cases followed by various other sciatica symptoms like numbness, muscular irritation, tingling sensation etc.

These may be a result of a more serious internal condition and it is imperative to consult a doctor immediately to be able to diagnose the correct reason for this condition. In rare cases, a surgery might be necessary too but in most cases the diagnosis consists of pain alleviating medication, analgesics and specially designed exercises that must be practiced with regularity in order to obtain sciatica relief.

The S-I joint is one of the most misunderstood areas of the human body. It has been the source of much controversy in the medical community for many years. Much of the debate relies on the fact that there are few reliable evaluation methods for the S-I joint.

Contrary to disc injuries, which can be evaluated using many types of diagnostic tools, examining the S-I joints has proved to be quite unreliable. Clinicians need to rely mostly on their experience rather than simple methods used to examine the low back. Most medical professionals do not acknowledge it as a source of pain and dysfunction. So patients leave a medical office with an incomplete evaluation and often times, an inaccurate diagnosis.

The S-I (Sacro-Iliac) joint is comprised of two bones; the sacrum and the ilium. You have two S-I joints (left and right). They are located basically where the spine meets the pelvis. Look for the two dimples in your low back. These two joints allow for very little movement overall as compared to other more prominent joints such as your hip or shoulder. The pelvic girdle is generally described as the two S-I joints, the pubic symphysis (pubic bone in front), the two hip joints, and the bottom two vertebrae (L4, L5).

What do you feel?

Pain located at or near the S-I joint on one side or both sides. Pain located in the low back, buttock, and/or groin area. Another common symptom of a S-I joint dysfunction is "sciatica". Sciatica is best described as a sharp, often shooting pain that begins in the buttocks and goes down the back of one leg. S-I joint dysfunction may also cause nerve irritation of the nerves that supply the groin or front of your thigh.

Other symptoms include:

Weakness in one leg or both legs; difficulty standing on one leg and raising the other leg (like marching)
Numbness and tingling in one leg (pins & needles)
A burning pain located near the "dimples"
Difficulty raising from a chair
Muscle discomfort in the buttock, hip, or low back (over 30 muscles attach to the pelvis; from the hip, buttock, thigh, low back.

How does this happen?

S-I joint dysfunction is usually caused by an imbalance in the muscles of the hips and glutes and it can also be caused by a fall or other traumatic event, such as a car accident.

A condition known as "hypermobility" may also predispose an individual to S-I joint dysfunction. "Hypermobility" is best described as a condition in which the joints have too much mobility. This condition generally affects women more than men. Different hormone levels present in women, specifically "relaxin", can influence hypermobility. This hormone is released to prepare the body for pregnancy, and its level in the blood changes throughout pregnancy process. It basically causes the ligaments to "relax" and allow for more movement to occur in the pelvic girdle region.

The best treatment options

First of all, the evaluation is critical to the success of treating a S-I joint dysfunction. Most dysfunctions can be treated with manual techniques that generally involve "mobilizations", "manipulations", or "muscle energy techniques". These techniques must be applied by a skilled medical professional, such as a physical therapist. These techniques can prove to be quite helpful when utilized appropriately following a thorough evaluation.

Once appropriate manual techniques have been administered, a comprehensive exercise program must be implemented to address the following areas, specifically muscle imbalances:

Lumbar stabilization program: strengthening abdominals and buttock muscles
Improve flexibility in lower extremity musculature

Some cases may only need a detailed exercise program that addresses the muscle imbalances. You can learn more about how to identify and address your muscle imbalances.

Some S-I joint dysfunctions may linger on for months and even years. Remember, if you are engaged in a current treatment plan with little to no improvement, seek other options. People with liver disease, people who drink alcohol heavily, and those taking blood – thinning medicines or NSAIDs should use acetaminophen with caution.

Ibuprofen

Ibuprofen is a pain reliever that combines anti-inflammatory properties with pain relief. People afflicted with rheumatoid arthritis generally have pain caused by inflammation and often benefit from aspirin or ibuprofen, the best know of which are Motrin and Advil. Unfortunately, ibuprofen products and aspirin typically cause some stomach discomfort if taken without food and have been linked to ulcers with long-term use.

What Causes Rheumatoid Arthritis?

Scientists are studying risk factors that increase the likelihood of developing a rheumatic disease. Some of these factors have been identified. For example, in osteoarthritis, inherited cartilage weakness or excessive stress on the joint from repeated injury may play a role. In rheumatoid arthritis, the combination of genetic factors that determine susceptibility and environmental triggers are believed to be important.

Nonsteroidal Anti-Inflammatory Drugs (NSAIDs)

This class of drugs includes aspirin and ibuprofen as well as Vioxx (rofecoxib), Celebrex (celecoxib), and Bextra (valdecoxib), which are classified as COX-2 inhibitors. Aspirin and ibuprofen are used to reduce pain and inflammation, as noted above, and may be used for both short-term and long-term relief in people with osteoarthritis but are more commonly used to treat symptoms of rheumatoid arthritis. The other drugs under NSAIDs are COX-2 inhibitors that work by blocking an enzyme known to cause an inflammatory response. COX-2 inhibitors had been approved for short-term use in the treatment of pain, and for longer-term use to treat the signs and symptoms of osteoarthritis and rheumatoid arthritis.

Side effects associated with NSAIDs include stomach irritation, negative impact on kidney functions, and are often associated with serious gastrointestinal problems, including ulcers, bleeding, and perforation of the stomach or intestine. As such, people over age 65 and those with any history of ulcers or gastrointestinal bleeding should use NSAIDs with caution. As with most medications, the longer a person uses NSAIDs, the more likely he or she is to have side effects. In addition, because of the way NSAIDs alter the way the body uses or eliminates other drugs, many other drugs cannot be taken when a patient is being treated with NSAIDs. It is important to check with your health care provider or pharmacist before you take NSAIDs in addition to other medications.

Zymosine is a all natural product that is formulated to assist in  joint pain relief . I personally was at my wits end and did not have a desire to get a cortisone shot.

1. ice after each activity that causes pain
2. heat before each activity to the affected area
3. ice massage works real well for increasing blood flow
4. aspirin, tylenol or advil for minor pain relief
5. mild exercise to strengthened the connective tissue

It was a major find on the part of a friend when he told me about zymosine. Both of us had some ailments (he had arthritis in the
ankles and wrists) and my elbow joint pain.
